Job Summary

We are seeking a skilled Community Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. The successful candidate will work in a dynamic environment, providing high-quality care to individuals with mental health needs.

Main Responsibilities

The post holder will be responsible for assessing care needs, developing and implementing individualized care plans, and evaluating the effectiveness of interventions. They will also participate in the setting of quality standards, auditing, and reviewing practice in line with current clinical guidance and policy.

The successful candidate will have experience of working with people experiencing mental health-related difficulties and will be able to demonstrate a commitment to evidence-based practice. They will also be able to work autonomously and as part of a multidisciplinary team.
