Job Description

We are seeking a skilled and compassionate Community Health Practitioner to join our established community nursing team in Erewash North. As a Nursing Associate, you will provide care for housebound patients in Ilkeston and the surrounding areas.

About the Role

You will be responsible for providing community-based individualized care, including long-term condition management, venepuncture, and medication administration. Additionally, you will complete various nursing assessments and follow treatment regimes.

This is an exciting opportunity for the successful candidate to progress and develop in the role. There may be an opportunity to apply for the Nursing Apprenticeship programme subject to successful completion of the probationary period.

Main Responsibilities
  • To work as a Nursing Associate, you must be passionate and enthusiastic about patient care, putting patients at the heart of everything you do.
  • You will work in a multi-professional team and need to be able to think on the spot and be empathetic.
  • You will be required to have access to a car to complete patient visits in the community and clinical settings.
About Us

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes, and increasingly, in people's own homes and via virtual consultations.

Requirements
  • A Registered Nursing Associate on the NMC register is essential.
  • Placement experience working within the 4 fields of nursing is also necessary.
  • You must be able to travel in your own vehicle across the area.
  • You should understand the scope of the role of the Nursing Associate in the context of the team and the organisation, and how the role contributes to service development.
  • You must be able to communicate effectively with patients, relatives, and carers, as well as all members of the multi-disciplinary team.

The estimated salary for this role is £25,000 - £33,000 per annum, depending on experience.
