This is an exciting opportunity for the successful candidate to progress and develop in their role, providing care for housebound patients in Ilkeston and the surrounding areas.

As a Community Health Practitioner, you will be responsible for providing individualised care to patients with long-term conditions, including venepuncture and medication administration, as well as completing various nursing assessments and following treatment regimes.

You will work as part of a multi-professional team, thinking on your feet and being empathetic towards patients. Effective communication skills are essential to work collaboratively with other staff and patients.

The post holder will need to have access to a car to complete patient visits in the community and clinical settings.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

To succeed in this role, you will need to be passionate about delivering high-quality patient care, with a strong understanding of the scope of the Nursing Associate role in the context of the team and the organisation.

It is also essential that you have excellent communication and organisational skills, with the ability to prioritise tasks effectively and manage your workload on a daily basis.

The salary for this role is approximately £25,000-£35,000 per annum, depending on experience.
