Health and Social Support Specialist

Premier Community is a leading provider of home community care services, and we are currently seeking experienced professionals to join our team as Personal Care Coordinators. In this role, you will play a vital part in delivering exceptional care and support to our clients, promoting their independence and well-being.
As a Personal Care Coordinator, you will be responsible for providing personalized care and support, including assistance with daily living activities, administration of medications, and recording visits live using our digital care planning system. You will also be expected to participate in comprehensive training and ongoing development opportunities, including fully funded qualifications in Health and Social Care.
The Role of a Personal Care Coordinator
- Assist clients in achieving their desired outcomes from our services, promoting their independence and well-being.
- Provide personal care assistance, including bathing and showering, as required.
- Administer medications as necessary, ensuring client safety and well-being.
- Record visits live using our digital care planning system, maintaining accurate records and facilitating seamless communication among our teams.
