We are seeking a highly skilled and experienced professional to join our team as an NHS Continuing Healthcare Practitioner. This is a challenging yet rewarding role that requires a high level of skill, expertise, and compassion.

About the Role

As a key member of our Care Coordination Team, you will be responsible for assessing patients' needs, developing personalized care plans, and ensuring seamless care delivery. You will work closely with patients, families, and other stakeholders to achieve the best possible outcomes.

The successful candidate will have a strong background in healthcare, with experience of working in a similar role or in a related field such as social work or nursing. You will be able to demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other stakeholders.

A key aspect of this role is the ability to monitor quality assurance and contribute to performance improvement initiatives. You will also be responsible for supporting new staff to complete mandatory training and ensuring that they have the necessary skills and knowledge to fulfill their roles effectively.

Base salary: £58,000 - £65,000 per annum
