Healthcare Pathway Specialist

We are seeking a highly skilled and experienced Care and Support Practitioner Lead to join our team at NHS Sussex ICB. As a key member of our healthcare pathway team, you will play a crucial role in ensuring the delivery of high-quality care and support services to our patients.
In this role, you will be responsible for leading the assessment and care planning process, working closely with multidisciplinary teams to identify individualized care needs and develop personalized care plans. You will also be involved in case management, ensuring that patients receive timely and effective care throughout their journey.
As a Care and Support Practitioner Lead, you will have the opportunity to work across various areas of our service, including audit, appeals, retrospective assessments, complaints, and assessment verification. We offer a supportive and collaborative work environment, where you will be encouraged to develop your skills and experience to work across the whole pathway, safely, confidently, and effectively.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work flexibly and as part of a team. You will be required to travel occasionally to carry out assessments and reviews, so access to transport is necessary. We offer a competitive salary package, including an estimated £43,000 - £55,000 per annum, depending on experience, plus benefits such as flexible working hours, annual leave, and professional development opportunities.
