Job Title: NHS Continuing Healthcare Practitioner

About the Role

We are seeking a skilled and experienced professional to join our team as an NHS Continuing Healthcare Practitioner. As a key member of our Care Coordination Team, you will play a vital role in ensuring that patients receive high-quality care and support.

The successful candidate will have a strong background in healthcare, with experience of working in a similar role or in a related field such as social work or nursing. You will be able to demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other stakeholders.

A key aspect of this role is the ability to assess patients' needs and develop personalized care plans that meet their unique requirements. You will also be responsible for monitoring quality assurance and contributing to performance improvement initiatives.

Key Responsibilities

Salary: £50,000 - £60,000 per annum (dependent on experience)
