About the Job

NHS Sussex ICB has an exciting opportunity for a highly skilled Senior Community Health Practitioner to join our Clinical Bank. This dynamic role will involve working across various areas of Continuing Healthcare, including audit, appeals, retrospective assessments, complaints, assessment verification, and more.

The successful candidate will have a strong background in continuing healthcare, with experience in patient assessment, care planning, and case management. As a Senior Practitioner, you will play a key role in developing and implementing effective care strategies, working closely with multidisciplinary teams to ensure optimal patient outcomes.

Responsibilities

  • Conduct thorough patient assessments, identifying physical and mental health needs and developing targeted care plans.
  • Develop and implement effective case management strategies, ensuring seamless coordination of care.
  • Maintain accurate records and reports, ensuring timely completion and submission as required.

What We Offer

NHS Sussex ICB offers a competitive salary range of £40,414 - £49,564 per annum, depending on experience. Requirements

  • Educated to degree level or equivalent experience in one of the following fields: Allied Health Professions (AHPs), Social Work, with subsequent up-to-date registration with the relevant regulatory body.
  • Minimum of 3 years' experience in continuing healthcare, with a proven track record of delivering high-quality patient assessments and care plans.
