About the Role

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our Adult Mental Health team. As a key member of our multidisciplinary team, you will provide high-quality, evidence-based care to adults with mental health needs, working within the THRIVE framework for service delivery.

The successful candidate will have a strong background in mental health nursing, with experience of working with adults with complex needs. You will be responsible for assessing, planning, implementing, and evaluating care for individuals, as well as providing supervision and guidance to junior staff members.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for adults with mental health needs
  • Provide supervision and guidance to junior staff members

  • Contribute to the development and implementation of care plans
  • Participate in multidisciplinary team meetings and contribute to decision-making
Requirements
  • Registered mental health nurse with a strong background in adult mental health
  • Experience of working with adults with complex needs
  • Strong assessment and planning skills
  • Ability to work collaboratively with other healthcare professionals
  • Excellent communication and interpersonal skills
