About the Role

We are seeking a highly skilled Mental Health Transition Specialist to join our team. As a key member of our Community Mental Health Team, you will play a crucial role in supporting young adults (18-25) with mental health difficulties to transition from children's services to adult care.

Our ideal candidate will have experience of providing care for individuals with mental health difficulties within a secondary care environment and working as a care coordinator/key worker. You will be able to work autonomously and recognize professional limitations, develop and implement collaborative care plans, and manage risk accordingly.

The salary for this role is estimated to be around £34,000 - £40,000 per annum, depending on experience. We offer a range of benefits, including flexible working requests, career development opportunities, and a supportive team environment.

Person Specification

We are looking for someone with a degree-level qualification or equivalent, and registration with a relevant professional body. Our ideal candidate will have at least one year's additional post-registration knowledge and experience, allowing them to be autonomous in their practice.
