About the Role

The successful candidate will be responsible for assisting in the development and implementation of integrated care pathways, including appropriate service user referrals and models of care. They will participate in case supervision to monitor named primary nurses' standards of service user care and documentation, ensuring that clinical environments promote patients' health, safety, and well-being.

Key responsibilities include giving/receiving regular clinical supervision as per Trust policy, ensuring the safe management of medicines, and adhering to the NMC code of professional conduct and Trust Medicines Code. The ideal candidate will also participate in the referral process, assess individuals in accordance with agreed care pathways, and complete quality reports where necessary.
