Disability Care Coordinator

We are looking for skilled and compassionate individuals to join our team as Disability Care Coordinators in our short breaks home, Pinewood. As a Disability Care Coordinator, you will be responsible for coordinating the care and support of young people with disabilities, ensuring that their needs are met and that they receive the best possible care.
You will work closely with a multidisciplinary team to provide individualised care plans and coordinate activities to meet the needs of each child. The salary for this role is £25,000 - £30,000 per year, depending on experience. We also offer a range of benefits including annual leave, pension scheme and employee assistance programme.
