Care Coordinator

We are seeking a highly skilled Care Coordinator to support our Chester Branch on a full-time basis working 37.5 hours per week.
As a Care Coordinator, your responsibilities will include coordinating the rota for care calls, maintaining accurate and detailed records, communicating with colleagues and clients regarding their planned visits, and supporting the on-call service on a rotational basis.
Key responsibilities include:
- Coordinating the rotas for our care staff.
- Effectively communicating rota changes to colleagues and clients.
- Supporting the on-call service on a rota basis.
- Building and maintaining positive relationships with our clients, family members, colleagues, and other professionals.
- Upholding the principles of Homecarers values.
- Ensuring the implementation of safe working practices.
