About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our Reablement team. As an Assistant Reablement Coordinator, you will play a vital role in promoting independence and supporting individuals in their homes to reach their full potential.

Key Responsibilities

  • Provide frontline care services, including Reablement, to individuals in their homes.
  • Work collaboratively with the Reablement team to develop and implement personalized care plans.
  • Support individuals in rebuilding their independence and confidence.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Conduct regular assessments and reviews to monitor progress and adjust care plans as needed.
