Job Title: Advanced Health Practitioner

We are seeking an experienced and skilled Advanced Health Practitioner to join our team in Surrey Heath. The successful candidate will provide a high level of care to patients with frailty or long-term conditions, working closely with the Integrated Care Team, Frailty MDT, and local GP teams.

The estimated salary for this role is £40,000 - £60,000 per annum, depending on experience. As a key member of our team, you will be responsible for devising effective personalized care plans for each patient, recognizing their individual needs and developing outcome-based plans that ensure appropriate pathways of care and communication.

To succeed in this role, you will need:

  • Maths and English GCSE's (C and above) or equivalent.
  • Registered Healthcare Professional, with either the NMC or HCPC.
  • Masters degree or equivalent experience gained by undertaking ongoing personal development and training.

You will also require significant relevant experience in frailty and proactive long-term condition management, as well as experience working with complex patient needs in a primary care/community setting. A car driver with a valid licence and flexibility to attend forms or training as learning needs are identified is essential.
