**Job Overview**

We are seeking an experienced and skilled Nurse Practitioner to join our team at Surrey Heath Community Providers Limited. The successful candidate will work clinically to provide evidence-based lower limb nursing services to patients, as part of the local GP Practices within the Federation/PCN.

About the Role:

  • Undertake holistic assessments of patients, including those with complex health needs, and develop appropriate plans of care with clear timelines for evaluation.
  • Assess, diagnose, implement, plan and review care needs for lower limb management in partnership with patients, carers and multi-professional teams.
  • Work with the wider multidisciplinary team to prevent hospital admission and reduce GP contact.
  • Demonstrate clinical expertise and competence within the specialty and work autonomously, managing own allocated clinics independently.

Person Specification:

  • Excellent communication and interpersonal skills.
  • Ability to advocate on patient, staff and safety or quality issues.
  • Leadership skills and ability to make decisions and prioritize tasks.
  • Clear understanding of Primary Care Services and current NHS developments.
  • Broad range of enhanced clinical skills and good working knowledge of clinical governance.

Qualifications:

  • NMC registration - Adult/General Nurse.
  • Post-registration qualification in relevant area of practice.
  • Teaching and assessing qualification, NMC Assessor and Supervisor status.

Salary:** £35,392 to £42,618 FTE, depending on experience
