Community Healthcare Specialist

We are seeking a highly skilled Community Practitioner to join our team at Surrey Heath Community Providers Limited. This is an exciting opportunity to work as part of a multi-disciplinary team providing urgent, on-the-day care to registered patients of GP practices in Surrey Heath.
About the RoleThe successful candidate will provide face-to-face minor illness management and work closely with the wider urgent care teams to deliver high-quality patient-centered clinical care. Additionally, they will be capable and confident in digital consultation where required.
Responsibilities- To provide highly skilled evidence-based nursing/AHP services to patients as an essential part of the healthcare provided by the GP Federation and local member GP practices.
- To support the development and delivery of policy and procedures, and provide clinical leadership to appropriate others as required.
- A Registered healthcare professional with the NMC or HCPC, with clinical supervision training and experience.
- Community or Primary Care specialist qualification and additional qualification in a specific long-term condition.
£38,000 - £46,000 per annum.
