Job Title: Mental Health Support Worker - Community Outreach

We are seeking a highly motivated and experienced Mental Health Support Worker to join our team in South Hertfordshire. The successful candidate will work closely with our care coordinators, psychiatry, and Clinical Nurse Specialist to support young people to engage with mental health services.

The ideal candidate will have experience of working with individuals with mental health difficulties and an interest in working with young people. As part of this role, you will receive regular supervision and support and will have opportunities to further develop your skills.

About the Role:

  • Lead on all referrals to the outreach team, offering consultation, creating a clear formulation of work to be undertaken, including a care plan.
  • Create a clear working plan around the intervention being offered to CYP, family, and clinical support.
  • Have a close working relationship with the CAMHS Nurse Specialist, team clinical lead, and nominated care coordinators.
  • Regularly meet with the supervisor and manager to ensure the post holder is supported to carry out the demands of the post.
  • Offer regular support and supervision to other support workers within the team.
  • Ensure that all care services provided are offered without prejudice regardless of age, creed, gender, and ethnicity.

Requirements:

  • Good standard of education (to include Maths and English GCSE grade C or above or equivalent)
  • Further Education or Diploma Training or NVQ in Health and Social Care
  • Experience of working with children and young people with mental health conditions in either acute or community settings
  • Experience of assisting in the assessment of CYP and implementing agreed care plans

Salary: £28,000 - £33,000 per annum
