Job Title: Ambulatory Outreach Nurse

We are seeking an enthusiastic and experienced Ambulatory Outreach Nurse to join our team at King's College Hospital NHS Foundation Trust.

About the Role

This is an exciting opportunity to work in a dynamic and fast-paced environment, providing high-quality care to patients in the ambulatory setting. As an Ambulatory Outreach Nurse, you will be responsible for leading a team of nurses and working closely with other healthcare professionals to deliver excellent patient care.

You will have the opportunity to work on a nurse-led unit, carrying out nurse-led discharges and predicting and preparing for discharges on the unit. You will also be expected to liaise and communicate well with patients, carers, and members of the Multi-Disciplinary Team (MDT).

Key Responsibilities
  • To assess, plan, implement, and evaluate the needs of patients on the ward on an ongoing basis, coordinating the care given by all members of the clinical team.
  • To ensure that practice is evidence-based and assist in the development of policies and protocols within the clinical area.
  • To contribute to the health education of patients and their families, ensuring patients receive appropriate advice and support to maintain a healthy lifestyle.
  • To actively support the implementation of the Trust nursing strategy and uphold the ethos of the local philosophy.
  • To manage the clinical workload effectively, ensuring care is prioritized appropriately.
Person Specification
  • 1st level Registered Paediatric Nurse
  • Mentorship programme or equivalent teaching qualification
  • Further relevant post-registration professional studies
  • Team building and working skills – able to lead a team to achieve results
