Job Title: Ambulatory Outreach Nurse

We are seeking an enthusiastic and experienced Ambulatory Outreach Nurse to join our team at King's College Hospital NHS Foundation Trust.

Job Summary:

This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality care to patients in the ambulatory setting. As an Ambulatory Outreach Nurse, you will be responsible for leading a team of nurses in providing care to patients in the ambulatory unit, ensuring a smooth transition from hospital to home.

Key Responsibilities:
  1. Lead a team of nurses in providing care to patients in the ambulatory unit.
  2. Develop and implement care plans for patients in the ambulatory unit.
  3. Collaborate with multidisciplinary teams to ensure seamless care transitions.
  4. Provide education and support to patients and their families.
  5. Contribute to the development of policies and procedures related to ambulatory care.
Requirements:
  • 1st level Registered Paediatric Nurse.
  • Mentorship programme or equivalent teaching qualification.
  • Further relevant post-registration professional studies.
  • Team building and working skills – able to lead a team to achieve results.
