Ambulatory Care Nurse

We are seeking an enthusiastic and experienced Ambulatory Outreach Nurse to join our team at King's College Hospital NHS Foundation Trust. As an Ambulatory Outreach Nurse, you will play a vital role in providing high-quality care to patients in the community, working closely with our multidisciplinary team to ensure seamless transitions between hospital and home.
Job SummaryThe successful candidate will have a strong background in paediatric nursing, with experience in ambulatory care and a passion for delivering patient-centred care. You will be responsible for assessing, planning, implementing, and evaluating the care of patients in the community, working collaboratively with other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Assess and plan the care of patients in the community, taking into account their individual needs and circumstances.
- Develop and implement individualised care plans, working closely with patients, families, and other healthcare professionals.
- Provide high-quality nursing care to patients in the community, including wound care, medication management, and education.
- Collaborate with other healthcare professionals to ensure seamless transitions between hospital and home.
- Contribute to the development and implementation of policies and procedures related to ambulatory care.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
To be successful in this role, you will need:
