Job Title: Ambulatory Outreach Nurse

We are seeking an enthusiastic and experienced Ambulatory Outreach Nurse to join our team at King's College Hospital NHS Foundation Trust. As an Ambulatory Outreach Nurse, you will play a vital role in providing high-quality patient care and ensuring a smooth transition from hospital to home.

Job Summary:

The successful candidate will be responsible for leading a team of nurses in the Ambulatory Unit, providing care to patients with complex needs. You will work closely with other healthcare professionals to ensure that patients receive the best possible care and support.

Key Responsibilities:
  • Lead a team of nurses in the Ambulatory Unit, providing care to patients with complex needs.
  • Work closely with other healthcare professionals to ensure that patients receive the best possible care and support.
  • Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Collaborate with other healthcare teams to ensure seamless transitions of care and minimize hospital readmissions.
  • Contribute to the development and implementation of policies and procedures that promote high-quality patient care.
Requirements:
  • 1st level Registered Paediatric Nurse
  • Mentorship programme or equivalent teaching qualification
  • Further relevant post-registration professional studies
