Job Title: Ambulatory Outreach Nurse

We are seeking an enthusiastic and experienced Ambulatory Outreach Nurse to join our team at King's College Hospital NHS Foundation Trust. As a key member of our Ambulatory Services team, you will play a vital role in delivering high-quality patient care and ensuring a smooth transition from hospital to home.

Job Summary

The successful candidate will be responsible for leading a team of nurses in the Ambulatory Unit, providing expert care to patients and their families. You will work closely with other healthcare professionals to ensure seamless communication and coordination of care. Your excellent communication and interpersonal skills will enable you to build strong relationships with patients, families, and colleagues.

Key Responsibilities
  • Lead a team of nurses in the Ambulatory Unit, providing expert care to patients and their families.
  • Work closely with other healthcare professionals to ensure seamless communication and coordination of care.
  • Develop and implement effective care plans to meet the needs of patients and their families.
  • Provide education and support to patients and their families to promote healthy lifestyles and self-management.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support.
  • Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
Requirements
  • 1st level Registered Paediatric Nurse qualification.
  • Mentorship programme or equivalent teaching qualification.
  • Further relevant post-registration professional studies.
  • Demonstrable clinical expertise, relevant to the area.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
