We are seeking a skilled Community Healthcare Professional to join our team in Long Stratton. This exciting opportunity will enable you to make a tangible difference in the lives of patients and contribute to delivering outstanding care in our local community.

The successful candidate will work predominantly independently, visiting patients in care homes across the area, addressing various care needs such as diabetes care, wound care, venepuncture, and catheter care. Our service operates 7 days a week, and we welcome applications from individuals who can demonstrate flexibility and a sound clinical basis.

Main Responsibilities
  • To provide high-quality interventions for patients with long-term conditions, palliative care, and rehabilitation needs, promoting quality of life and independence where possible.
  • To work collaboratively within the integrated team to facilitate early discharge from hospital and prevent unnecessary admissions.
  • To engage effectively with healthcare professionals, statutory, and non-statutory agencies to deliver seamless, integrated services to our users.

Salary: £24,071 - £25,674 per annum (pro rata), depending on experience.
