We are seeking an experienced Community Healthcare Assistant to join our team at Norfolk Community Health and Care NHS Trust. This is a fantastic opportunity to work with our dedicated nursing team in Long Stratton, providing specialist care to patients in care homes across the area.

About the Role

As a Community Healthcare Assistant, you will be responsible for visiting patients, addressing their varied care needs, and contributing to the delivery of interventions for patients with long-term conditions. You will also work within the integrated team to facilitate early discharge from hospital and prevent unnecessary admission to hospital.

We estimate the salary for this role to be around £24,000 - £30,000 per annum, depending on experience.

Key Responsibilities:

  • To visit patients in care homes and address their varied care needs, including wound care and catheter care.
  • To contribute to the delivery of interventions for patients with long-term conditions, palliative care, and rehabilitation needs.
  • To work collaboratively with healthcare professionals and statutory/non-statutory agencies to provide a seamless service to our patients.
