We are proud to be part of the Transformation Unit, driving innovation and excellence in community healthcare.

We are currently looking for a dedicated Healthcare Assistant to join our community nursing team in Long Stratton. This role involves working in a fast-paced environment, providing person-centered care to patients with complex needs, including diabetes management, wound care, and catheter maintenance.

The successful candidate will be responsible for planning and prioritizing visits, documenting patient records, and ensuring timely inputting of data into SystmOne. Strong communication skills, empathy, and tact are essential in this role.

Main Duties
  1. To apply competency-based treatment techniques and care to patients with specific conditions under the supervision of a qualified practitioner.
  2. To plan and prioritize own visits to delegated patients, ensuring documentation and timely SystmOne inputting.
  3. To ensure patient-held records are completed for each visit and report findings to qualified practitioners.

This role requires a strong foundation in health and social care, with a focus on delivering high-quality care that promotes independence and dignity.

Salary: £24,071 - £25,674 per annum (pro rata), depending on experience.
