Job Overview

We are seeking a skilled Community Healthcare Professional to join our team in Long Stratton. The successful candidate will be responsible for visiting patients in care homes and addressing various care needs.

Salary: £25,500 - £31,020 per annum (dependent on experience)

Job Description

The role involves contributing to the delivery of interventions for patients with long-term conditions, palliative care, and rehabilitation needs. You will work within an integrated team to facilitate early discharge from hospital and prevent unnecessary admission.

You will work closely with healthcare professionals and statutory/non-statutory agencies to provide a seamless service to our users.

We offer part-time and full-time hours available.
