We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Homeless Health Care Team in Southampton. The successful candidate will have a strong background in mental health care, with experience working with individuals from diverse backgrounds.

This is an excellent opportunity for a qualified RN Mental Health Nurse, Social Worker, or Allied Health Professional to make a real difference in the lives of vulnerable adults. As a key member of our team, you will be responsible for providing high-quality care coordination and advocacy services to support individuals who are experiencing homelessness.

You will have a good understanding of mental health policies and practices, as well as experience in assessing and formulating individual intervention plans. You will work collaboratively with colleagues to develop and implement care plans that meet the unique needs of each service user.

The role requires excellent communication and interpersonal skills, with the ability to build strong relationships with service users, their families, and partner agencies. You will also contribute to weekly multidisciplinary team meetings and participate in supervision arrangements to support the development of your skills and knowledge.
