The Southampton Primary Care Network is seeking a dedicated professional to fill the role of Care and Training Practitioner. This position is part of a growing primary care team that delivers care to diverse populations in Southampton. This Band 7 role focuses on integrating primary care services and mental health support to improve health and wellbeing outcomes for high-utilising patient groups. The successful candidate will become part of a collaborative team working towards maximising life opportunities for patients and their families.

We are looking for an enthusiastic and creative individual with a flexible approach, strong understanding of risk assessment, crisis planning, and the recovery model. This role requires excellent communication skills, a multi-disciplinary mindset, and the ability to adapt to changing situations.

Key Responsibilities:

  • Deliver a primary care-based clinical operations approach that integrates primary care services and mental health support.
  • Work collaboratively with the primary care team to develop and implement strategies that improve health and wellbeing outcomes.
  • Provide training and support to staff members on psychologically informed interventions.
  • Maintain accurate records and reports as required.
