Key Responsibilities

The Hospice Palliative Care Specialist will be responsible for:

  • Providing support to Hospice at Home Carers via telephone or in-person, ensuring they have the necessary resources and guidance to perform their duties effectively.
  • Conducting face-to-face assessments of patients, including care planning and risk assessments, to ensure accurate documentation and patient-centered care.
  • Collaborating with the Deputy Hospice at Home Manager to monitor and review cases, carrying out quality assurance audits to maintain high standards of care.
  • Performing care visits when necessary to support safe service delivery and avoid unmet needs, adjusting care plans based on ongoing assessments and patient needs.
  • Communicating regularly with Nurse Specialists and the wider Multi-Disciplinary Team to ensure timely and optimal care for patients and families.

The successful candidate will have a strong understanding of palliative care principles and practices, with experience of working in a community setting.

This role comes with a competitive salary of £26,530 - £29,114 per annum, pro rata, plus excellent benefits, including generous holiday allowance, refer-a-friend scheme, and industry-leading training and development opportunities.
