We are seeking a highly skilled Clinical Nurse Specialist to join our Hospice Community Specialist Palliative Care Team. The ideal candidate will have experience in providing palliative care to patients with life-limiting illnesses and their families. Responsibilities include:


  1. Working within the NMC Scope of Professional Practice and Code of Conduct;
  2. Identifying professional objectives and developmental needs;
  3. Maintaining a caseload of patients under the direct supervision of a Community Clinical Nurse Specialist in Palliative Care;
  4. Attending regular clinical team meetings, locality meetings and Multidisciplinary team meetings to discuss patient management;
  5. Assisting the Community Clinical Nurse Specialist team in the continuous assessment and evaluation of patient needs;
  6. Working with the clinical multidisciplinary team to provide ongoing assessment, effective symptom management and monitoring of patient care;
  7. Demonstrating effective communication with colleagues and healthcare professionals to facilitate continuity of care for patients with life-limiting illnesses;
  8. Maintaining accurate patient records, including inputting patients onto the electronic end of life care register Universal Care Plan;
  9. Demonstrating skills in working with families facing loss and identifying risk factors that require communication with the Hospice Social Work and Bereavement Service;
  10. Participating in clinical audit as part of the hospice audit programme.
