Job Overview

We are currently recruiting for a Social Prescribing Specialist to join our team at Principal Medical Limited. The successful candidate will be responsible for supporting individuals to connect with community resources and services that promote their health and wellbeing.

In this role, you will meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. You will help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.

You will also help people maintain or regain independence through living skills, adaptations, enablement approaches and simple safeguards. This may involve introducing them to community groups, activities, and statutory services, ensuring they are comfortable. Where people may be eligible for a personal health budget, you will help them to explore this option as a way of providing funded, personalised support to be independent.

The ideal candidate will have a commitment to reducing health inequalities and proactively working to reach people from all communities. They will also have experience of working within primary care and possess excellent communication skills.

We offer a competitive salary of £24,000-£26,000 per annum FTE, depending on experience.
