About the Job

We are looking for a skilled Community Development Specialist or Social Prescribing Link Worker to join our team in Oxfordshire and Northamptonshire. The successful candidate will work closely with local agencies, community groups, and individuals to develop strong links and encourage referrals to our social prescribing service.

The ideal candidate will have experience of working directly in a community development context, adult health and social care, learning support, or public health/health improvement. They will be able to build relationships across various organisations and have a proven track record of partnership/collaborative working.

You will work proactively to develop strong links with local agencies, community groups, and statutory services to raise awareness of social prescribing and its benefits. Your role will involve providing referral agencies with regular updates about social prescribing, including training for their staff and how to access information to encourage appropriate referrals.

In this exciting opportunity, you will work collectively with all local partners to ensure community groups are strong and sustainable. You will also work with commissioners and local partners to identify unmet needs within the community and gaps in community provision, supporting the development of new groups and services where needed.

Key Responsibilities
  • Develop strong links with local agencies, community groups, and statutory services to promote social prescribing and encourage referrals.
  • Work proactively to raise awareness of social prescribing among referral agencies, providing them with regular updates and training opportunities.
  • Support community groups and VCSE organisations to receive referrals and help them develop supportive relationships with local partners.
  • Identify unmet needs within the community and gaps in community provision, working with commissioners and local partners to develop new groups and services.
