Key responsibilities include:

  • Acting as a core member of the Specialist Palliative Care multidisciplinary team.
  • Facilitating effective patient management through collaborative working.
  • Promoting the Five Priorities of Care and supporting staff development.
  • Attending weekly team meetings and contributing to patient assessment and care planning decisions.
  • Prescribing and reviewing medication as a non-medical prescriber.
  • Demonstrating advanced practice in nursing care planning, implementation, and evaluation.
  • Providing Level 2 psychological support for patients and their carers.
  • Offering specialist expertise to healthcare professionals.
  • Leading clinical practice and upholding high standards of holistic care.

Essential requirements include:

  • Significant experience in palliative and end-of-life care.
  • Up-to-date knowledge of current statutory frameworks and national policies.
  • Evidence of holistically assessing patients in an acute hospital setting.
  • Audit participation or leadership experience.

This role offers a competitive salary of approximately £43,041 - £69,756 per annum, depending on experience, plus benefits including pension scheme, life assurance, and annual leave.
