- Acting as a core member of the Specialist Palliative Care multidisciplinary team.

- Promoting the Five Priorities of Care and supporting staff development.
- Attending weekly team meetings and contributing to patient assessment and care planning decisions.
- Prescribing and reviewing medication as a non-medical prescriber.
- Demonstrating advanced practice in nursing care planning, implementation, and evaluation.
- Providing Level 2 psychological support for patients and their carers.
- Offering specialist expertise to healthcare professionals.
- Leading clinical practice and upholding high standards of holistic care.
Essential requirements include:
- Significant experience in palliative and end-of-life care.
- Up-to-date knowledge of current statutory frameworks and national policies.
- Evidence of holistically assessing patients in an acute hospital setting.
- Audit participation or leadership experience.
This role offers a competitive salary of approximately £43,041 - £69,756 per annum, depending on experience, plus benefits including pension scheme, life assurance, and annual leave.
