Role Overview:
This is an exciting opportunity for a Nurse Adviser to join Chemistree Solutions in Lancashire, Blackburn. As a specialist nurse advisor with an intimate understanding of national treatment guidelines, you will work together with patients and practices to assist healthcare professionals to deliver the most up-to-date care for their patients.
The role involves responsibility for a defined geographical area and so demands excellent planning & organisational skills along with confident driving. The ideal candidate will be a patient-focused NMC registered nurse who can demonstrate experience of working in Diabetes.
Key Responsibilities:
- Deliver a unique nursing programme that focuses closely on driving best practice, delivering excellent patient care, and improving patient outcomes.
- Work in partnership with the NHS in primary care to support healthcare professionals/patients to make informed decisions about treatment pathways in Diabetes.
- Collaborate with patients and practices to assist healthcare professionals to deliver the most up-to-date care for their patients.
Requirements:
- A minimum of 3 years experience (post registration) managing diabetes within primary care.
- Knowledgeable and confident in the provision of education, support and advice to people with Type 2 Diabetes.
- Skills and knowledge in the optimisation of oral hypoglycaemics.
- IT literacy, to include primary care systems.
- Current (NMC) Registration and Diabetes Diploma or Equivalent.
