About the Role

We are seeking an experienced Occupational Therapist to join our Community Services Team. The successful candidate will be responsible for completing OT assessments for individuals discharged from short-term beds in acute hospitals, with the goal of facilitating a timely return home.

The ideal candidate will possess a good understanding of community equipment and environmental adaptations, as well as experience with complex moving and handling. Independent working skills and current registration with the HCPC are also essential requirements.

A valid UK driving licence and access to a car are necessary for this role. In addition, the hourly rate for this position is up to £32ph, depending on experience.

Job Responsibilities:

  • Completing OT assessments for individuals discharged from short-term beds
  • Facilitating timely returns home through effective support and guidance
  • Developing and implementing individualized care plans
  • Maintaining accurate records and reporting as required

Requirements:

  • Bachelor's degree in Occupational Therapy or related field
  • Current registration with the HCPC
  • Valid UK driving licence and access to a car
  • Good understanding of community equipment and environmental adaptations
  • Experience with complex moving and handling
