Job Summary:
We are looking for a highly skilled and dedicated individual to work as a Care Manager in Adult Social Care. The role involves working with adults who have care and support needs, ensuring their safety and well-being.
About the Role:
As a Care Manager, you will be responsible for assessing the needs of adults, developing and implementing care plans, and coordinating services to ensure their safety and well-being. You will work closely with other professionals, including healthcare staff, police, and voluntary partners.
Requirements:
To be successful in this role, you will need:

- Experience in adult social care, preferably in a safeguarding role\
- Knowledge of adult care legislation and procedures\
- Good communication and interpersonal skills\
- Ability to work autonomously and as part of a team\
- Effective record-keeping skills\
- Familiarity with IT systems, particularly electronic social care records\
- A full driving license (as travel around the county will be required)\
- An enhanced Disclosure and Barring Service (DBS) check\
- Able to work in a hybrid environment, with some time spent in the office and others working from home\
