  • You will be responsible for safeguarding adults' referrals, triaging, and responding to risk, while ensuring compliance with the Care Act 2014 and internal policies.
  • Your duties will include undertaking strengths-based outcome-focused assessments of need, updating electronic social care records, and working collaboratively with agencies and partners.

To succeed in this role, you will require:

  • Experience in working with adults who have care and support needs.
  • Knowledge of Adult Care Legislation and procedures.
  • Good communication skills.
  • Experience of effective and efficient record keeping.
  • The ability to work as part of a team and autonomously.
