We are looking for a highly skilled Care Manager to join our Adult Social Care team in Rutland, UK.

This fixed-term position, up to 18 months, offers a competitive salary range of £31,067 to £41,511 per annum, based on a 37-hour workweek.

  • As a Care Manager, you will be responsible for safeguarding adults' referrals, triaging, and responding to risk, while ensuring compliance with the Care Act 2014 and internal policies.
  • Key duties include undertaking strengths-based outcome-focused assessments of need, updating electronic social care records, and working collaboratively with agencies and partners.
  • You will work in a solution-focused environment, empowering adults and their carers in safeguarding processes.

To be successful in this role, you will require:

  • Experience in working with adults who have care and support needs.
  • Knowledge of Adult Care Legislation and procedures.
  • Strong communication skills and experience of effective record keeping.
  • The ability to work collaboratively as part of a team and autonomously.
