Job Opportunity
We are seeking a skilled Care Manager to join our Adult Social Care team in Rutland on a fixed-term basis, working 37 hours per week.

Key Responsibilities:
- Develop individualized care plans in collaboration with clients, their carers, and other relevant stakeholders.
- Maintain accurate records and reports, ensuring compliance with relevant legislation and policies.
- Collaborate with healthcare professionals, social workers, and other agencies to ensure comprehensive support for clients.
- Conduct regular reviews and assessments to monitor progress and make necessary adjustments to care plans.
