We are seeking a skilled and compassionate Care Manager to join our team at Rutland County Council.
The role of Care Manager is varied and requires working with some of the most at-risk members of our community. Our goal is to empower adults and their carers to achieve their goals in a supportive and solution-focused environment.
Key Responsibilities:
- To coordinate and deliver high-quality care services to vulnerable adults
- To work collaboratively with other professionals to ensure effective care planning and delivery
- To maintain accurate records and reports to meet council requirements
Required Skills and Qualifications:
- Degree in Social Work or related field
- Experience in adult social care
- Excellent communication and interpersonal skills
