We are looking for a highly skilled Care Manager to join our team at Rutland County Council. In this role, you will be responsible for working with adults aged 18+ who require safeguarding and support.

You will undertake strengths-based outcome-focused assessments of need, monitoring, coordination, and review of care plans, risk assessments, and protection plans, working in an integrated way with colleagues and other agencies to ensure the effective support of vulnerable adults.

To succeed in this role, you will need experience in working with adults who have care and support needs, knowledge of Adult Care Legislation and procedures, and strong communication skills.

Salary: £31,067-£41,511 per annum (dependent on experience). This is a fixed-term position up to 18 months.
