Care Manager - Safeguarding

The role of Care Manager in Adult Social Care is varied, working with some of the most at-risk members of our Rutland community. We support adults and their carers to be empowered to achieve their chosen outcomes throughout the safeguarding process, working in partnership with others to prevent and protect adults from abuse and neglect.

You will be working with adults aged 18+ and will deal with cases requiring you to be resilient and empathetic.

About you

To be successful in the role you will need to have:

  • Experience in working with adults who have care and support needs
  • Knowledge of Adult Care Legislation and procedures
  • Good communication skills
  • Experience of effective and efficient record keeping
  • The ability to work as part of a team and autonomously

About the role

As a Care Manager your duties will include:

  • Working in a team and being responsible for safeguarding adults referrals, triaging, and responding to risk.
  • Work in line with the Care Act 2014, LLR Safeguarding Procedures and internal policies to ensure the safeguarding of adults in Rutland.
  • To provide information, advice and guidance for referrals that need signposting to other partners and universal services.
  • Undertaking strengths-based outcome focused assessments of need including monitoring, coordination, and review of care plans, risk assessments and protection plans.
  • Updating and maintaining electronic social care records
  • To work in an integrated way with colleagues and other agencies, including but not limited to Police, Health services, providers and voluntary partners.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.

Please note a full driving licence will be required to enable the postholder to travel around the county.
