Lead Community Manager

About the Role
">- As a key member of our team, you will be responsible for leading the development and implementation of systems and processes to ensure effective flow of patients between hospital and community settings.
- You will work closely with clinical and operational teams to ensure that transitions from hospital to community are safe and timely, utilizing a Home First and Discharge to Assess Model.
- We offer an exciting opportunity to work on delivering excellent care to our patients, ensuring that we deliver effective, safe, and timely discharge services.
- In this role, you will lead on the implementation and embedding of the Transition From Hospital business case and associated workforce model, supporting system partners to develop safe and timely pathways for patients to leave hospital.
- Our ideal candidate will have experience in managing complex projects, with a proven track record of delivering meaningful and sustained change in large teams or acute, community, or social care settings.
- With strong leadership skills and the ability to build strong networks and alliances, you will be able to act assertively to challenge lack of progress and resolve conflict, driving change and improving patient outcomes.
Salary Information
">The salary for this position is approximately £45,000 - £55,000 per annum, depending on experience.

Requirements
">- To be successful in this role, you will need to have a postgraduate qualification in a relevant field, such as healthcare management or nursing.
- Experience of complaints management and resolution, as well as knowledge of risk management processes and governance, are essential requirements for this position.
- Strong communication and interpersonal skills are also required, with the ability to work under pressure and manage multiple complex projects to tight deadlines.
Physical Skills
">- Full UK driving license and access to own vehicle are essential for this role, with visibility across wards and departments and community settings required.
