Community Treatment Specialist

Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a highly motivated and enthusiastic clinician to join our Older Adult Community Treatment Team. As a Band 6 Community Practitioner, you will play a crucial role in coordinating mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions, focusing on care homes.
About the Role
As a Community Practitioner, you will work in a highly supported environment and be provided with training opportunities to develop your knowledge and skills. Your strong commitment to delivering high-quality, safe patient care is essential. You will be responsible for assessing and managing casework of service users with substantial and complex needs, developing formulation-focused care plans using a psychosocial approach, and providing clinical and psychologically based interventions.
Key Responsibilities
• Assess and manage casework of service users with substantial and complex needs
• Develop formulation-focused care plans using a psychosocial approach
• Provide clinical and psychologically based interventions
• Provide specialist advice to service users, carers, staff, statutory agencies, third sector partner agencies, and the public
• Provide clinical supervision to staff as delegated by the Clinical Lead and Pathway Manager

Salary Range
The estimated salary for this position is £38,890 - £44,997 per annum.
