Community Care Specialist

Swansea Bay University Health Board invites applications from experienced Community Care Specialists to join our team. As a key member of our Primary Care and Therapies Group, you will be responsible for assessing individual patient care needs, escalating concerns to the correct team members, and delivering high-quality patient care.
About the Role- You will work as part of a multi-disciplinary team to provide comprehensive care services.
- Able to speak Welsh is desirable but not required.
The ideal candidate will have NMC registration, adult branch, and experience in mentorship and managing individuals with long-term conditions. The salary for this position is estimated to be around £35,000 - £50,000 per annum, depending on experience.
