Social Care Practitioner

Derbyshire County Council seeks an experienced and dedicated Social Care Practitioner to join its High Peak Adult Social Work Team.
The ideal candidate will have a strong understanding of assessing and reviewing service user needs in community and residential settings, with experience working within budgets and knowledge of outcome-focused assessments.
A key aspect of the role will be creating person-centred and strengths-based assessments and support plans, commissioning services to enable individuals to meet their outcomes, and promoting a home-first culture.
Excellent communication and interpersonal skills are essential for building effective relationships with service users, their families and carers, as well as other professionals in a multi-disciplinary partnership approach.
The selected candidate will be expected to work independently, prioritising tasks effectively and managing time efficiently to meet deadlines and deliver high-quality results.
