Community Response Care Coordinator

**Job Overview:**
We are seeking a highly motivated and organized Community Response Care Coordinator to join our team at Derbyshire Community Health Services NHS Foundation Trust. As a Community Response Care Coordinator, you will be responsible for coordinating community response teams and ensuring seamless delivery of patient care services.
**Responsibilities:**
* Coordinate community response teams to ensure timely and effective delivery of patient care services* Develop and maintain strong relationships with healthcare professionals and stakeholders* Manage patient caseloads and coordinate care plans* Conduct regular audits and quality checks to ensure high standards of patient care* Participate in ongoing training and professional development to enhance skills and knowledge* Liaise with family members and caregivers to provide updates on patient progress and involve them in decision-making processes as needed
**Requirements:**
* 2+ years experience in a healthcare or social care setting* Strong organizational and time management skills* Excellent communication and interpersonal skills* Ability to work effectively under pressure and prioritize tasks* Valid driver's license and access to reliable transportation* Basic computer skills and proficiency in Microsoft Office Suite* Willingness to participate in ongoing training and professional development
