**Job Description:**
We are seeking a skilled Care Support Specialist to join our team at Derbyshire Community Health Services NHS Foundation Trust. As a Care Support Specialist, you will play a vital role in providing high-quality care to patients in their own homes.
**Responsibilities:**
* Provide personalized care and support to patients, promoting independence and well-being* Work collaboratively with healthcare professionals to deliver effective patient care* Maintain accurate records of patient interactions and progress
**Requirements:**
* NVQ Level III in care or equivalent qualification* Experience working in a healthcare setting* Strong communication and interpersonal skills* Ability to travel across the county to attend essential training and other required training for your role
