Main Responsibilities
  • Assess and identify the needs of patients with diabetes, developing and implementing individualized care plans to optimize their health and well-being.
  • Provide education and support to patients, relatives, and carers, empowering them to manage their diabetes effectively and make informed decisions about their care.
  • Collaborate with healthcare professionals to ensure seamless transitions of care, promoting continuity and coordination of services.
  • Contribute to the development and implementation of diabetes care pathways, ensuring that patients receive high-quality, evidence-based care.
  • Participate in quality improvement initiatives, using data and evidence to inform practice and drive improvements in patient care.
Requirements
  • Registered Nurse (RGN) with a first-level degree and experience in caring for patients with diabetes.
  • Sound knowledge of diabetes care and management, including insulin therapy, medication management, and lifestyle interventions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, prioritizing tasks and managing workload effectively.
